PIIB Application

To apply for affiliation complete the downloadable application and attach the required documents for a quick response.

In addition to the application PIIB needs the following for approval:

  • 2 years financials – balance sheet and income statement
  • Company experience – just print and sign the company release attached to the application
  • 3 months Trust Account statements
  • Brief bio of agency and each principal

Once approved additional information will be required to complete company appointment paperwork:

  • Personnel and Automation Data form which we will supply
  • Copies of agency and all staff licenses
  • E&O dec page
  • Bond and latest payment voucher
  • Voided check from Operating and Trust Accounts
  • DMV Authorization letter
